If you have not yet received your letter in the mail, or the email sent June 12th, please click on the "Labs & Documents" tab in your portal, and then "General Resources for All Patients". The important message is within the document titled "Clinic Closing Letter". Thank you!

*Please note that medical record requests will continue to be processed for the forseeable future (on Mondays), and requests will be handled in the order they are received. If there is a specific date you need your records transferred by due to an appointment with another provider, please leave a message in the portal so we can do our best to process your request by the date they are needed.

Set your password
